Hello, I am an italian firefightert! Sorry for the english!
I hope these things:
Starting with a small barracks and enlarge over time. Possibility to buy trucks of the same type but different (better pumps, cafés, better engine, higher ladder).
Most interventions: saf, nbcr, urgent opening door (not urgent), release elevator and so on.
Camion con acqua limitata, nozzles
Truck with limited water, different nozzless and courses
more vehicles (eg platforms bronto skylift)
More equipment eg tools to secure a car.
training for firefighters.
firefighters with different characteristics
wrong Emergency calls by civil
Map obscured until the arrival on site of means.
Map obscured until the arrival of firefighters on the spot

Better Maps. Meaning more variations and the ability to change or import vehicles and models from other mods.
The ability to mod is a must I feel for any future releases.

Better fire dynamics, like flashovers, backdrafts, building collapses, Class B, C, D, K fires. Different police dynamics too, like the ability to resolve a situation without arresting someone.

What is that?
The game I showed is a simulator made for learning institutions to train real firefighters, police and paramedics. It has some in-depth simulation and seems to represent real world scenarios quite nicely. Regular users can't buy or download this, though.
The reason I put it in the thread was to give an example of how great Emergency 5 -could- be. Look at the first video and imagine the same thing happening in Emergency - I'm amazed by the thought.

That's cool, I've had some firefighting simulators that were intended to be used for training exercises for firefighting and I found that most were a failure at graphics and interactivity. I think if video game designers and instructors worked together the results would be better for all.

The simulator is nice, but too much for a game though. I do see where certain aspects could be implemented if game developers would stop being so cheap and just give us better control over editing.
It would be nice if the editor would work similar to that simulation editor. But I'm not sure how it would work outside of a SET environment with hills etc...
The game needs to expand the randomness of various accidents. As a user we can of course do this by creating multiple accidents in the same area, but some accidents should be able to be created where ever. Example: Head on collision with X amount of additional vehicles, Rear end collisions, Vehicle running off the road into a tree, pole, ditch, building.
I'd like to not be able to SEE what the emergency is until someone arrives on the scene.
Dispatch SETS would be nice too rather than individuals. House fire you can setup a dispatch set and when the call comes in you dispatch it. Same with business building fires. Car accidents. Hostage.
One of my main issues is lane change. This has to be in the game. If an ambulance is parked outside a building the traffic should be able to go around it.
The same with accidents. The flares and cones have to dictate traffic. I'd like to see a traffic control when you put too officers in place. Select them both and choose between opposite direction, intersection etc.. When you set them up they handle the traffic AUTOMATED rather than manual like we are doing now.
Better Emergency vehicle awareness by vehicles. Driving right up on an EMERGENCY vehicle that has its lights on makes no sense.
Vehicles that move to allow emergency vehicles to get by.
Even though we can do this with MOD some things should be done with an editor.
I like the dispatch of firemen, but it needs to be expanded by DEFAULT.
In other words... Firemen can dispatch to hook up to fire truck, fire extinguishers, only the firemen hook and the emt/fireman handles injuries, car accident dispatch.
Same with ambulance. dispatch with the stretcher or without. Police can dispatch for accident so they automatically put on the vest and automatically take them off entering the car. Police that automatically setup landing zones for helicopters. Swat various dispatches. Make the shields mean something. As of now it is just decoration. Various arrest such as laying down, hands on vehicle, etc...
Expand the engineer role and vehicles. Same with fire chief. Maybe have him investigate some fires requiring action.
I'd like to make additional police parking zones as simple as adding a parking sign in a parking lot rather than drawing out a zone. And some of the zones are pretty much the same so it should be a simple drag and drop or click and then user placement. All typical... universal defined... zones and things should be like this.
Expand demonstration. I'd like to have the kind where they block traffic, competing demonstrators for/against.
More weather related issues. Earthquake okay, but some flooding... snowfall and effects of icing, etc...
Effects of storms such as power outage which of course effects traffic signals etc...
Patrols... with a purpose and vision. Which means more crime and make it depend on how the user handles it. No community presence then more crimes related to that such as stolen vehicles and break ins. Drug activity. Prostitute activity.
How about vandalism? Looting???
No predicable escape routes.
Realistic spreading of fires. Some of these fires spread way too fast. Even in a drought they would not spread like they do in a blink of an eye.
Cars that blow up should have casualties rather than just putting out the fire and towing.
Make vehicles with options actually mean something. Mobile hospital with deploy able tent should mean something like they can care for more serious injuries on site or something other than just a visual thing.
Towing needs a huge overhaul. Huge trucks and buses should not fit on the tow trucks.
Bigger map of course.
Multiple entrances to buildings.
Multiple floors. They don't have to go crazy with it, but have something.
More police weapons. Swat too. Ability to go to roof of buildings. Firemen with ladders. Better control over ladder trucks. Annoying that some will not extend to put out fires when it is well with in reach and not obstructed.
Less crowded UI.

Also it would be nice if the missions were blended into the default map given. So instead of playing it simply as a mission just make the even happen and we have to respond to it. Multiple criminals too. Driver and robber. Gangs. etc...

To be honest i would like thi things on the EM5:
1)Police Units (vehicles) with the hability to change the direction of the cars... to turn them back where they came like the L.A Mod
2)All tipe of units of the Emergency Services not just Police, Medics, Fire Dept and Enginers. Example: on the tree of the Police Forces they are: Border Police, Highway Police, Riot Police, Transit Police etc... and i think on some countries the Military-Police ... they are (at least here on my country and i think part of the world) they patrol around every Border of the states and the Country... (i don't know Europe, but here on Latin-America we have that type of Police..)
3)Better Graphics
4)More Units, because the Police, Fire Dept, Enginers and Medics... have more than only Road Units.. they have Helicopters and in the case of the Fire Dept, they have Boats and Airplanes... i don't know if all the countries count with that.. but part they have them...
5)Police Barriers & Traffic Cones. The hability to put Fences & Cones in the angle u want to re-direct the Vehicles and People around it... that would be a great idea..
6)a Big Map and Units more faster (because the units never go slow.. they go like airplanes in the sky when they are called.. no matter how heavy the vehicle is.. they go as faster they can)
i think those would be good ideas for the game...
P.D: The Map would be better if it's about a Real Location of the World..

Emergency 5 could use several things,
1. Better AI, for Emergency crewmembers, civilians, and criminals. Armed suspects just shoot the vehicles and don't notice the police officers who can arrest them. Civilians should also get out of the way for emergency vehicles and go around when there is one blocking there path. Also an ambulance should not drive backwards up a whole street (I speak from experiance here).
2. Better police chases, when your playing a mod without the chase command or the origianal game the only way to stop a fleeing cartheft is with a spikestrip or a helicopter. That is unrealistic, in real life a suspect does not just abandon there car when a police helicopter starts folowing them. The suspect should go around the unit blocking its path, turn around, or plow through instead of just sticking a rifle out its window and shooting the vehicle blocking it until it explodes. They should add in commands for your patrol cars to attempt the pit manuver or something like that. When the vehicle is stoped the officers should be able to pull the driver out.
3. More realistic firefighting, like a limited water supply and the need to connect vehicles to hydrants. There should also be other materials to fight fire with than water. Backdrafts would be a nice adittion. The ability to cut holes in a buildings roof and to break windows for ventilation should be added too. Also firefighting range of the latter truck is rather low, it has a really long latter and yet it can only extinguish fires really close to it.
4. Better sirens, in the origonal emergency 4, the vehicles sirens turn off after about 5 seconds. Like in most mods, the emergency vehicles need the ability to turn the siren on/off. Also the civilians don't pull over, they just stop, causing your vehicles to go around them!
5. Individual stations, there should be several fire stations, police stations, and a hospitile (the number of these depending on the map size). You should also be able to upgrade these stations to have more room for personel, vehicles, and equipment.
6. More than "mission failed", In the missions added by the LA mod, if you fail the mission, it says,"Command has been given over to the FBI or the USFS or something like that for the folowing reason" instead of "mission failed". Emergency 5 should include this.
7. More types of guns! As you know in normal Em4 your regular police officers are unarmed. They should have pistols at all times and mabey a rifle or shotgun in the trunk of the patrol car they can get. The "marksman" should have the automatic weopons and a pistol as a backup. The "sharpshooter" can remain the same. Police in Em5 should be able to run while shooting instead of kneeling to shoot and should be able to take cover behind things. Bullets should have more effect on people and less effect on vehicles. There should be RPG's for extreme situations like for taking down a stolen tank or apc (A mission or freeplay event I hope Em5 has). Shots should also miss there targets sometimes.
8. Damage, If a vehicle takes damage, it should show it with broken windows, dents, smoking engine, and other visual effects. It should also drive slower. When a vehicle is destroyed, the wreck should be on fire and need to be extinquished. Anyone in the vehicle should need to be cut out with the jaws of life. Also the blast mark under the vehicle should not vanish until long after the wreck is removed or in a mission, not at all. Building should be able to colapse if the fire is not put out in time. If that is in freeplay, you should then be able to see the building get rebuilt.
9. Buildings, buildings should have multiple floors like in Em1. Police sharpshooters should be able to shoot from in the building through an upperstory window. If a shootout ocures in a building, stuff in the room should get destroyed (like furniture, or stuff like that).
10.The military, in missions when the military is present they should either be under your command or help you. It is redicules if they stand around doing nothing while your services do all the work.
11. Last of all, the editor. The editor should be more user-freindly and able to create missions entirly in the editor without having to have scripting knolege and editing other documents. The models should be in a format compatable with google sketchup, or even better, the ability to edit both models and textures in the editor.
That is alot of the stuff that would be nice to see in Emergency 5.
